SERVICE ANIMALS

Open Jar Studios complies with ADA regulations, which allows ADA accredited service dogs trained to provide assistance to individuals with disabilities into the facility.  A service animal means any dog that is individually trained to do work or perform tasks for the benefit of an individual with a disability, including a physical, sensory, psychiatric, intellectual, or other mental disability. 

Under Title III of the ADA, Emotional support animals, comfort animals, and therapy dogs are NOT considered service animals. 

Open Jar Studios requires that the following form be filled and signed prior to entry to the premises.

SERVICE ANIMAL REGISTRATION

REQUIREMENTS:

Service animals will remain in close control of the owner the entire time and owner understands that OJS has many shared areas and owners accept full responsibility for the care and maintenance of the animal both in work areas and the shared areas, as follows:

  • Owners will not allow the animal to be a nuisance in any shared area or private studios on the premises.

  • Owners will ensure that the animal's behavior does not pose a direct threat to the health or safety of others, including not barking or growling at other people or acting out of control. Owner understand if the animal's behavior is disturbing other guests it may be excluded.

  • Owners will not allow the animal to be unleashed or unrestrained in any shared area or private studios on the premises.

  • Owners recognize that other clients may be allergic or afraid of my service animal, which also requires accommodation and will ensure the animal stays within my close care.

  • Owners understand that if they do not follow the guidelines, the service animal may be excluded from the premises. 

Dog Owners understand that OJS will strive to protect the safety of the service animal, but that there are many risks involved that are out of OJS’s control. These include but are not limited to: injury or death to the service animal; injury or death to another animal, or a person, caused by the service animal; property damage caused by my service animal; ingestion of items causing injury or death to my service animal. Owner understands that there are inherent risks that exist. Owners also understand that it is their responsibility to be truthful in completing the above information and disclosing any past history of aggressive behavior which the service animal has.

ASSUMPTION OF RISK 

Dog Owners understand that the behavior of domestic animals, including the Assistance Dog, is unpredictable and that there are inherent risks associated with domestic animals, including the Service Animal, which include, but are not limited to, bodily injury or the spread of disease as a result of being in close proximity to the Service Animal. Owners further acknowledge and understand that personal injury (including death) or property damage may occur as a result of certain animal behaviors, including, but not limited to, biting; jumping; running; quickly walking; pulling on leashes; not responding to commands; stepping on individuals; unpredictable reactions to sounds; or sudden movements due to unfamiliar objects, persons or other animals, including other animals.

In the event of any damage caused by my service animal to the facility, Dog Owners agree to be financially responsible for any such damage.

Dog owners understand that  any damage caused by the behavior of domestic animals, including the Assistance Dog, is unpredictable and that they will be financially responsible to restore in full any damage caused to the facility by their service animal, which include, but are not limited to, scratches, bite marks, or animal waste on seating, glass tables, mirrors, specialized dance floors, walls, concrete, air conditioning, heating units. Owners agree to cover the cost of restoring any damaged property caused by my Service Animal.
